    Review and Accept Waiver

    Agreement and Liability Release

    This is an agreement between ForeKidz, Inc., a Minnesota nonprofit, and Golf Student and their parent/legal guardian for Golf Student’s participation in 2026 ForeKidz summer program teaching participants how to play golf.

    ForeKidz promises. At no cost to Golf Student and in consideration for the promises in Paragraph 2 and the liability release in Paragraph 3, ForeKidz will:

    • Provide students with golf clubs to use during the summer.

    • Provide two weekly group lessons with a golf pro at the Keller Golf Course driving range.

    • Provide Golf Student tokens to the Keller Golf Course driving range.

    • Possible 9-hole rounds of golf at Keller Golf Course with an experienced golfer and/or golf pro.

    Golf Student promises. In consideration for the opportunity to participate in the ForeKidz summer program, Golf Student promises:

    • To treat the loaned golf clubs with respect and to take care of them.

    • To return the loaned golf clubs to ForeKidz at the end of Golf Student’s participation in the program.

    • To treat instructors, supervisors and other participants in the ForeKidz summer program with respect.

    • To follow directions from the program instructors and supervisors.

    • To follow the rules of Keller Golf Course.

    • To treat fellow golfers with respect.

    Golf Student acknowledges that if they fail to honor these promises, ForeKidz will have the right to remove them from the summer program.

    Liability Release. In signing this Agreement, Golf Student and their parent/guardian acknowledge that certain risks are inherent in the summer program. These include the risk of personal injury (from, for example, being hit by a ball or club) or damage to property. In exchange for being permitted to participate in the ForeKidz summer program, Golf Student and their parent/guardian agree as follows:

    Released parties: This Release applies to claims against the following Released Parties: ForeKidz, Inc. and its officers, directors, employees, agents and volunteers.

    Release: I release the Released Parties from liability for any claim I or Golf Student may have at any time for i) personal injury of any kind Golf Student may suffer or ii) any damage to my or Golf Student’s property that is caused by any Released Party’s negligence during or in connection with the ForeKidz summer program. I understand that in signing this Agreement and Release, I am giving up certain legal rights, including the right to sue the Released Parties for injuries or losses that fall within this Release.

    Parental Consent and Release

    By checking the box, I am the parent or legal guardian of the Golf Student named above, and I consent to Golf Student’s participation in the event. I have read the above Release, and to the extent permitted by law, on behalf of myself and my child, I agree to be bound by Paragraphs 2(a)-(f) and 3(a)-(b) above and release the Released Parties from claims that I or Golf Student may have that fall within the scope of the Release in Paragraph 3(a)-(b). I further consent to ForeKidz use of images of Golf Student to promote or market its programs.
